At Hygenerate, we operate as a hybrid clinical practice.

Our goal is to provide thoughtful, systems-based medical care while maintaining transparency around fees, insurance eligibility, and scheduling policies.

 

We believe patients should understand both the clinical and financial aspects of their care before scheduling.

Our Care Model

Hygenerate is a nurse practitioner–led practice providing structured metabolic care, hormone management, men’s health services, stress physiology support, and immune/inflammatory evaluation.

 

We operate as a hybrid model:

 

• Some visit categories may be eligible for insurance coverage

• Other services are offered on a self-pay basis

• Coverage depends on medical necessity and individual plan benefits

 

We encourage patients to review their specific insurance benefits prior to scheduling.

Because our care pathways vary in scope and complexity, visit structure and fees differ accordingly.

Booking Deposits

To protect dedicated provider time and maintain scheduling integrity:

 

• A $50 non-refundable booking deposit is required for all new patient visits

• The deposit is applied toward your visit fee

• Deposits reserve extended clinical appointment time

• Missed appointments or late cancellations outside policy may result in forfeiture

 

This structure allows us to maintain longer, more focused visits for each patient.

Insurance Eligibility

Select visit types — including metabolic optimization, weight management, hormone evaluation, and certain men’s health services — may be eligible for insurance coverage depending on:

 

• Individual insurance plan benefits

• Deductible and copay status

• Medical necessity

• Diagnosis coding

• Provider licensure within your state

 

Coverage cannot be guaranteed prior to claim processing.

 

Patients remain responsible for:

 

• Deductibles

• Copays

• Coinsurance

• Any non-covered services

 

We are happy to provide documentation or superbills when appropriate.

Understanding Visit Categories

Hygenerate offers structured care across multiple clinical pathways. While all visits reflect systems-based thinking, the scope, depth, and time allocation vary by category.

 

Metabolic optimization, hormone evaluation, and men’s health visits focus on targeted physiologic systems within defined clinical parameters.

 

Functional medicine visits involve comprehensive multi-system evaluation, extended history review, and complex care coordination across endocrine, immune, inflammatory, gastrointestinal, and stress physiology domains.

 

Visit fees reflect time, scope, and complexity — not quality of care.

These visits are intentionally focused. The appointment block includes provider time for documentation, prescription management, and order entry to ensure care remains accurate and on schedule.

Comprehensive laboratory interpretation requires functional medicine follow-up.
 

 Lab Clarification Appointment (20 minutes)

 

$100 

 

For patients who have follow-up questions regarding previously provided laboratory interpretation and care plans.

 

This visit does not include new comprehensive analysis.

Functional Medicine Care

 

Self-pay only

 

Functional medicine visits are comprehensive, systems-based, and designed for multi-system complexity including immune dysregulation, inflammatory conditions, chronic fatigue, dermatologic conditions, gastrointestinal disruption, and complex hormonal-metabolic interplay.

 

Comprehensive laboratory testing is typically performed at the start of care.

 

 

Comprehensive Functional Medicine Intake (60 minutes)

• Detailed systems-based history

• Timeline mapping

• Prior record review

• Laboratory strategy

• Initial therapeutic framework

 

Extensive written laboratory interpretation is provided as part of your care plan.

Includes:

 

• In-depth lab analysis

• Pattern refinement

• Supplement and therapeutic adjustments

• Ongoing systems-based recalibration

 

 

Functional Medicine Follow-Up (45 minutes)

Reserved for:

 

• Advanced case progression

• Multi-system recalibration

• Complex laboratory interpretation

• Significant care plan modification

Self-Pay Services

Certain services are provided on a self-pay basis.

 

Self-pay visit fees vary depending on visit type, complexity, and clinical scope. Specific fee information is provided within the scheduling portal prior to final booking.

 

This ensures transparency while preserving individualized care planning.

Licensure Requirements

Patients must be physically located in a state where the selected provider is licensed at the time of the visit.
